对比增强血管造影双期扫描在头颈部血管狭窄的临床应用 韦 寅 ，陈 鹏 ，谭 璨 ，李燕华 ，严健明 （广西梧州市人民医院放射科 广西 梧州 ５４３０００） 【摘 要】 目的 探讨一次增强完成头颈部双期扫描在头颈部血管狭窄中的应用价值 。方法 对 ５１例可疑头颈部动脉 狭窄的患者分别行 ３D CE‐M RA双期和 DSA检查 ，对成像结果进行相关性比较 。结果 与 DSA相比 ，３D CE‐M RA动脉 期及 ３D CE‐M RA双期诊断头颈部动脉狭窄灵敏度 、特异度 、阳性预测值 、阴性预测值 、总符合率分别为 ９０ ．８１％ 和 ９０畅８１％ 、９２ ．９６％ 和 ９５ ．６２％ 、７１ ．７９％ 和 ８０ ．３８％ 、９８ ．０９％ 和 ９８ ．１４％ 、９２ ．６％ 和 ９４ ．８３％ ；诊断头颈部动脉轻度 、中度 、重 度狭窄 、闭塞的符合率分别为 ：８１ ．１０％ 和 ８９ ．８６％ ；６０ ．６５％ 和 ７４ ．０７％ ；９１ ．３０％ 和 ９５ ．４５％ ；７８ ．５８％ 和 ９１ ．６７％ 。结论 ３D CE‐M RA双期（动脉期后加做第二期）扫描 ，额外耗时少 ，不用增加对比剂剂量 ，不用增加患者额外经济负担 ，还能提 供比 ３D CE‐M RA动脉期更丰富的影像信息 ，更高的诊断准确性 。 【关键词】 颈部动脉疾病 ；对比剂增强 ；磁共振血管成像 中图分类号 ：R７４３ ；R４４５ ．２ 文献标识码 ：A 文章编号 ：１００６‐９０１１（２０１５）０８‐１３３３‐０５ Comparative analyses of dual‐phase contrast‐enhanced MR angiography （CE‐MRA） in the clinical application of head and ca‐ rotid artery stenosis WEI Y in ，CHEN Peng ，TA N Can ，L I Y an‐hua ，YA N Jian‐ming Department o f Radiology ，The Peop le’s Hosp ital o f W uzhou ，Guangx i ５４３０００ ，P ．R ．China 【Abstract】 Objective To explore the application of dual‐phase ３D CE‐M RA in head and carotid artery stenosis ．Methods The dual‐phase ３D CE‐M RA and digital subtraction angiography （DSA） were performed in ５１ cases suspected with head and carotid artery stenosis ．We made a correlated comparison concerning the diagnostic results of arterial‐phase ３D CE‐ M RA ，dual‐phase ３D CE‐MRA and DSA ．Results Compared with the results of DSA ，which was regarded as the “gold standard” ，the sensitivity ，specificity ，positive predictive values ，negative predictive values and the total corresponding rate of arterial‐phase and dual‐phase ３D CE‐M RA in head and carotid artery stenosis were ９０ ．８１％ and ９０ ．８１％ ，９２ ．９６％ and ９５ ．６２％ ，９８ ．０９％ and ９８ ．１４％ ，７１ ．７９％ and ８０ ．３８％ ，９２ ．６％ and ９４ ．８３％ ，respectively ．The consistent rates in di‐ agnosis on mid‐stenosis ，moderate‐stenosis ，severe‐stenosis and complete occlusion were ８１ ．１０％ and ８９ ．８６％ ；６０ ．６５％ and ７４ ．０７％ ；９１ ．３０％ and ９５ ．４５％ ；７８ ．５８％ and ９１ ．６７％ ．Conclusion Compared with the arterial‐phase ３D CE‐M RA
