Nonagenarians and centenarians represent a quickly growing age group worldwide. In parallel, the prevalence of dementia increasessubstantially,buthowtode?nedementiainthisoldest-oldagesegmentremainsunclear.Althoughtheideathattherisk ofAlzheimer’sdisease(AD)decreasesafterage90hasnowbeenquestioned,theoldest-oldstillrepresentapopulationrelatively resistanttodegenerativebrainprocesses.Brainagingischaracterisedbytheformationofneuro?brillarytangles(NFTs)andsenile plaques(SPs)aswellasneuronalandsynapticlossinbothcognitivelyintactindividualsandpatientswithAD.Innondemented casesNFTsareusuallyrestrictedtothehippocampalformation,whereastheprogressiveinvolvementoftheassociationareasin thetemporalneocortexparallelsthedevelopmentofovertclinicalsignsofdementia.Incontrast,thereislittlecorrelationbetween thequantitativedistributionofSPandADseverity.Thepatternoflesiondistributionandneuronallosschangesinextremeaging relativetotheyounger-old.IncontrasttoyoungercaseswheredementiaismainlyrelatedtosevereNFTformationwithinadjacent componentsofthemedialandinf
