MoniqueCHaak6? and Abstract Background:Womenwithashortcervicallengthinmid-trimesterpregnancyhaveahigherriskofpretermbirth andthereforeahigherrateofneonatalmortalityandmorbidity.Progesteronecanpotentiallydecreasethenumber ofpretermbirthsandlowerneonatalmortalityandmorbidity.Previousstudiesshowedgoodresultsof progesteroneinwomenwitheitherahistoryofpretermbirthorashortcervix.However,itisunknownwhether screeningforashortcervixandsubsequenttreatmentinmidtrimesterpregnancyiseffectiveinlowriskwomen. Methods/Design:Weplanacombinedscreenandtreatstudyamongwomenwithasingletonpregnancywithouta previouspretermbirth.Inthesewomen,wewillmeasurecervicallengthatthestandardanomalyscanperformed between18and22weeks.Womenwithcervicallength≤30mmattwoindependentmeasurementswillberandomly allocatedtoreceiveeithervaginalprogesteronetabletsorplacebobetween22and34weeks.Theprimaryoutcomeof thistrialisadverseneonatalcondition,definedasacompositeoutcomeofneonatalmortalityandseveremorbidity. Secondaryoutcomesaretimetodelivery,pretermbirthratebefore32,34and37weeks,daysofadmissioninneonatal intensivecareunit,maternalmorbidity,maternaladmissiondaysforpretermlabourandcosts.Wewillassessgrowth, physicalconditionandneurodevelopmentaloutcomeofthechildrenattwoyearsofage.
