三维超声在胎儿胼胝体发育不良诊断中应用价值[摘要] 目的 探讨三维超声在胎儿胼胝体发育不良(agenesis of the corpus callosum,ACC)产前诊断中的应用价值。方法 应用三维超声对49510例胎儿进行常规二维和实时三维超声检查。结果 49510例胎儿中检出ACC 25例,其中合并消化系统异常2例,心脏发育异常4例,骨骼异常4例,合并其他畸形者均伴羊水过多。结论 三维超声与普通超声相比能提供更丰富的诊断信息,有效的提高了超声图像的分辨力,解决了早期显示胎儿胼胝体困难的难题,是评价胎儿脑部发育的安全、经济、有效的影像学方法,具有重要的应用价值。 [关键词] 三维超声; 胼胝体发育不良; 产前诊断 [中图分类号] R729 [文献标识码] A [文章编号] 1673-9701(2009)27-104-02 Significance of Three-dimensional Ultrasonography in Diagnosis of Fetal Agenesis of the Corpus Callosum GUO Yue GUAN Yunping XIANG Yushi PU Xue Department of Ultrasound,Shenyang Obstetrics and Gyniatrics Hospital,Shenyang 110014,China [Abstract] ObjectiveTo assess the value of three-dimensional ultrasonography in the prenatal diagnosis of fetal agenesis of the corpus callosum(ACC). MethodsWe examined 49510 fetuses by using two-dimensional ultrasound(2DUS)and real-time three-dimensional ultrasound(3DUS). ResultsTwenty ACC in 49510 cases were found,including 2 cases of the abnomal digestive system,4 cases of the malformation of cardiovascular system,4 cases of skeletal abnormalities,and those of other malformation with polyhydramnios. Conclusion3DUS provides more diagnostic information than 2DUS. 3DUS evaluates ultrasound image resolution effectively,and solves thedifficult issue in early showing of the corpus callosum. Therefore,it is a safe,economical and effective imaging method in the assessment of fetal brain development,with an important practinal clinical value. [Key words]Three-dimensional ultrasound; Agenesis of the corpus callosum; Prenatal diagnosis 胼胝体发育不良是一种少见的脑部异常,有报道在所有的尸检病例中约占1∶19000,在所有的智力发育迟缓的孩童中占2.3%[1]。2005年1月～2009年3月来我院进行常规超声检查的49510例孕妇中检出ACC 25例,本文就4年来我院利用三维超声对胎儿胼胝体发育不全的诊断作一回顾性分析。 1 资料与方法 1.1 一般资料 共检出ACC 25例,孕妇年龄21～42岁,平均26.3岁。孕周20～41周,平均27.5周。 1.2 仪器 采用Philips iu22三维彩色多功能显像仪,凸阵探头,频率3.5～5MHz;三维容积探头,频率4～7MHz。 1.3 超声检测方法 孕妇仰卧位,于腹部多切面常规做二维超声检查,观察颅内及面部结构。对于头颅异常者进行三维超声检查,立体、直观并动态地观察颅内结构。 2 结果 25例ACC的超声所见见表1。 3 讨论 胼胝体(corpus callosu
