米索前列醇联合杜冷丁及利多卡因在人工流产术中无痛的临床效果观察 　　摘要：目的：观察米索前列醇联合杜冷丁及利多卡因用于人工流产术的镇痛、扩宫效果。方法：选择自愿行无痛人流术而无手术禁忌症，无米索前列醇应用禁忌症的早孕妇女360例，随机分为A组（米索前列醇术前舌下含化，杜冷丁术前肌肉注射联合利多卡因术时宫旁注射）和B组（笑气组），每组180例观察两组镇痛效果、人流综合症发生率、术中宫口扩张的情况、术中出血量及子宫收缩情况。结果：A组术中镇痛、扩张宫颈有效率明显优于B组，人工流产综合症发生率低于B组，两组相比差异有统计学意义（P0.05）。结论：无痛人流术前4～6h舌下含化米索前列醇可达到良好的扩张宫颈作用，杜冷丁术前肌肉注射联合利多卡因术时宫旁阻滞可增强无痛人流术的镇痛效果，缩短手术时间。米索前列醇联合杜冷丁及利多卡因联合应用于无痛人工流产安全、简便、有效，尤其适用于基层医院推广应用。 　　关键词：无痛人流；米索前列醇；杜冷丁；利多卡因 　　Joint demerol misoprostol and lidocaine in the clinical effect of intraoperative painless induced abortion 　　Wang Yan 　　Abstract：Objective：to observe the joint demerol misoprostol and lidocaine used for induced abortion operation effect of analgesia， expanding palace.Methods： choose voluntarily painless abortion surgery without surgery contraindications， misoprostol applied contraindications to pregnant women and 360 cases were randomly divided into group A （sublingual misoprostol preoperative containing， demerol preoperative intramuscular joint lidocaine near the palace when taking） and group B （laughing gas group）， 180 cases in each group to observe the analgesic effect of the two groups， the incidence of abortion syndrome， intraoperative bleeding amount of palace mouth of expansion， and uterine contractions. Results： intraoperative analgesia， expansion cervical efficient group A was better than group B， lower incidence of induced abortion syndrome than group B， compared two groups was statistically significant difference （P0.05）. Conclusion： painless abortion preoperative 4～6 h sublingual containing the misoprostol can achieve good expanding cervix， demerol preoperative intramuscular joint lidocaine art palace beside block can enhance the analgesic effect of painless abortion operation， shorten operation time.
