腹腔镜微创治疗胃部胃肠间质瘤近期临床效果研究 　　【摘要】 目的：分析腹腔镜以及传统术式胃切除术治疗胃部胃肠间质瘤（gastrointestinal stromal tumors， GISTs）的近期疗效。方法：回顾性分析2011年2月-2015年12月本院采用胃切除术、楔形切除术、外翻切除术治疗的胃部胃肠间质瘤患者55例，腹腔镜组24例采用腹腔镜手术治疗，开腹组31例采用开腹术手术治疗。结果：腹腔镜组手术时间、术中出血量、恢复普食时间、术后住院时间分别为（70.4±23.5）min、（25.4±10.3）mL、（5.2±1.1）d、（7.0±2.5）d低于开腹组（140.3±52.1）min、（89.4±70.5）mL、（8.1±2.1）d、（12.4±7.3）d，两组比较差异均有统计学意义（P0.05）。两组完整切除率均为100%，腹腔镜组并发症发生率54.17%低于开腹组93.55%，两组比较差异有统计学意义（P0.05），两组均未见严重的并发症。结论：腹腔镜微创治疗胃部胃肠间质瘤优势明显，尽管不能提高切除效果，但患者恢复速度更快，并发症发生率显著下降。 　　【关键词】 胃肠间质瘤； 胃部手术； 腹腔镜； 临床疗效 　　【Abstract】 Objective：To analyze recent curative effect of laparoscopic and conventional surgery gastric resection of gastric gastrointestinal stromal tumor （GISTs）.Method：55 patients of gastrointestinal stromal tumor were given gastrectomy， wedge resection，and resection of evaginate from February 2011 to December 2015，they were divided into laparoscopic group of 24 cases who were given laparoscopic surgery treatment and open group of 31 cases who were given open surgery operation treatment.Result： The operation time， intraoperative blood loss， recovery time and postoperative hospital stay time in laparoscopic group wese respectively （70.4±23.5）min，（25.4±10.3）mL，（5.2±1.1）d and （7.0±2.5）d lower than open group of （140.3±52.1）min，（89.4±70.5）mL，（8.1±2.1）d and （12.4±7.3）d，there were statistically significant difference （P0.05）.The rate of complications in laparoscopic group was 54.17% lower than that of open group 93.55%，the difference was statistically significant（P0.05）.There were no serious complications in the two groups.Conclusion：Laparoscopic minimally invasive treatment of gastric gastrointestinal stromal tumor minimally features obvious advantages，however it cannot improve the effect of resection， but patients recover are faster， the incidence of complications is much lower. 　　【Key words】 Gastrointestinal stromal tumor； Stomach surgery； Laparoscope； Clinical curative effect 　　First-author’s address：Dalian Central Hospital，Dalian 116023，China 　　d
