脉冲式注入造影剂法对判断超声造影中输卵管远段病理性与痉挛性阻塞诊断作用 　　【摘要】 目的：探讨脉冲式注入造影剂法对判断子宫输卵管四维超声造影的输卵管远段病理性阻塞与痉挛性阻塞的诊断作用，为患者的妊娠愿望得以实现而提供技术性的帮助。方法：选取2015年 　　1月-2016年12月在本院诊断为不孕症的患者58例，于月经干净3～7 d后行子宫输卵管四维超声造影检查，均采用脉冲式的冲击疗法打造影剂，与腹腔镜下美兰液通液检查法进行比较，对比两种检查方法下患者输卵管远段的通畅性差异。结果：四维超声造影检查显示，通畅65条，不通35条，通而不畅16条。腹腔镜下美兰液通液检查显示，通畅55条，不通40条，通而不畅21条。以超声造影作为诊断金标准，腹腔镜下美兰液通液法的灵敏度为60.78%，特异度为53.85%，诊断符合率为56.90%。结论：脉冲式注入造影剂法对判断子宫输卵管四维超声造影的输卵管远段病理性阻塞与痉挛性阻塞有明确的诊断作用，值得推广。 　　【关键词】 脉冲式； 子宫输卵管四维超声造影； 病理性阻塞； 痉挛性阻塞 　　【Abstract】 Objective：To investigate the diagnostic value of impulse type injection contrast agent method on pathological and spasmodic obstruction of distal segment of fallopian tube by uterus and fallopian tube four-dimensional contrast-enhanced ultrasound，and to provide technical help for the realization of the pregnancy desire of the patients.Method：58 cases of infertility diagnosed in our hospital from January 2015 to December 2016 were selected，after the menstrual cleaning 3-7 d，the uterus and fallopian tubes were examined by four-dimensional contrast-enhanced ultrasound，and the pulse type shock therapy was used to make the shadow agent，and the results were compared with that of the laparoscopic methylene blue liquid examination，compared the two methods of examination of patients with tubal distal patency difference.Result：Four-dimensional contrast-enhanced ultrasound showed that 65 pieces of unobstructed，35 pieces of blocked and 16 pieces of obstruction and obstruction.Meilan liquid penetration test under laparoscope showed that 55 pieces of unobstructed，40 pieces of blocked and 21 pieces of obstruction and obstruction.The sensitivity of Meilan liquid penetration test under laparoscope was 60.78%，the specificity was 53.85%，and diagnostic accordance rate was 56.90%.Conclusion：Impulse type injection contrast agent method has a definite diagnostic effect on pathological and spasmodic obstruction of distal segment of fallopian tube by uterus and fallopian tube four-dimensional contrast-enhanced ultrasou
