小儿肺炎支原体肺炎临床剖析 　　[摘要] 目的 探讨小儿肺炎支原体肺炎的临床表现特点及感染规律,为小儿MP肺炎的诊治提供科学依据。方法 对本院750例呼吸道感染患儿进行肺炎支原体检测,调查肺炎支原体肺炎流行情况,并对入院的118例确诊为肺炎支原体肺炎的患儿进行实验室检查及并发症的回顾性分析。结果 呼吸道疾病患儿抗-Mp-IgM阳性率为60.0%(450/750),女性患儿阳性率(68.0%),高于男性患儿(54.7%,χ2=48.1,P0.01)。婴幼儿MP肺炎以气促、喘息为主要临床表现,肺部?音多见,发热并不明显,肺外并发症少;年长儿患儿的MP肺炎以发热、咳嗽为主,肺部体征不明显,肺外并发症多。51.7%的患儿有肺外合并症。结论 小儿MP肺炎在不同年龄组有不同的临床表现,早期无明显的阳性体征,实验室检查结果呈现多样性改变,肺外合并症多,因此对疑似患儿应常规检测MP-IgM抗体,有利于早期诊断,减少并发症的发生。 　　[关键词] 呼吸道感染;肺炎支原体;IgM抗体;并发症;实验室检查 　　[中图分类号] R725.6 [文献标识码] A[文章编号] 1673-9701(2010)14-38-03 　　 　　Clinical Analysis of Mycoplasma Pneumonia 　　HU Xiaohua 　　Pediatrics Department of Taojiang County Peoples Hospital,Taojiang 413400,China 　　 　　[Abstract] Objective To investigate the mycoplasma pneumonia infection of clinical characteristics and rules,providing a scientific basis for the diagnosis and treatment of children with MP pneumoniain. Methods750 cases of children with respiratory tract Mycoplasma pneumoniae wereinvestigated for the prevalence of Mycoplasma pneumoniae pneumonia,laboratory tests and complications of 118 patients who were diagnosed as mycoplasma pneumonia wereretrospectively analyzed. Results Respiratory disease in children with anti-Mp-IgM positive rate was 60.0%(450/750), female children with positive rate(68.0%) twas higher than male patients(54.7%, χ2=48.1, P0.01). MP pneumonia in infants and young children had shortness of breath,wheezing as the main clinical manifestation. pulmonary rales weremore common,fever is not obvious and extrapulmonary had few complications;while older children with MP pneumonia had fever,cough and extrapulmonary had more complications. 51.7% of the children had extrapulmonary complications. Conclusion MP pneumonia in children in different age groups have different clinical manifestations. It has no significantly positive early signs,and laboratory test results showed the diversity changes in more than extrapulmonary complications,so it should be routine testing in children suspected of MP-IgM antibodie
