寰枢关节脱位手术治疗及并发症处理 Surgical treatment of atlantoaxial subluxation and management of complications 第四军医大学西京医院神经外科脊柱组 Xituan Ji Department of Neurosurgery, Xi-Jing Hospital, Fourth Military Medical University, Xi’an (710032) China Introduction The traditional management of atlantoaxial subluxation is odontoidectomy by oral-pharynx approach and cervical – occipitalia bony fusion. This method brings too much damage to this kind of patients. Now the concept operation has been introduced that is direct posterior restoration and fixation . The advantage: posterior approach only, pure posterior approach, direct intraoperative restoration and fixation, without cervical traction or tracheotomy. History ➢ Abumi, et al 1999, reported 24 cases . Direct posterior restoration and screw-rod (plate) internal fixation technique for the treatment of atlantoaxial subluxation. ➢ Goel A make a lot of work in atlantoaxial subluxation. ➢ In the neurosurgical field ,the technique has been used in many hospitals including Bei Jing, Guangdong , Tianjin ,Xi’an, Chengdu, Hangzhou etc. since 2004.Such as Wang Chao ,Yin Qingshui(orthopedics),Jian Fengzeng,Qiao Guangyu,Fan Tao,Chen Zan,Wu Hao et al.(neurosurgery) History in Xijing hospital The first case was treated by prof Fei zhou using the internal fixation technique in our department in Feb ,2008. After then, more than 600 cases have been successfully cured in our hospital. We reviewed and reported these cases in National Neurosurgery Conference in Nan Chang, 2009. From then on, the novel therapeutic strategy has been widely studied and carried out in many national hospitals, such as Xinan Hospital in Third Military Medical University, the first affiliated hospital of Zhejiang University, and Huaxi Hospital.
