Arti?cialintelligencehassigni?cantlycontributedintheevolutionofmedicalinformaticsandbiomedicine,providingavariety oftoolsavailabletobeexploited,fromrule-basedexpertsystemsandfuzzylogictoneuralnetworksandgeneticalgorithms. Moreover,familiarizingpeoplewithsmartphonesandtheconstantlygrowinguseofmedical-relatedmobileapplicationsenables completeandsystematicmonitoringofaseriesofchronicdiseasesbothbyhealthprofessionalsandpatients.Inthiswork,we proposeanintegratedsystemformonitoringandearlynoti?cationforpatientssu?eringfromheartdiseases.CardioSmart365 consistsofwebapplications,smartphonenativeapplications,decisionsupportsystems,andwebservicesthatallowinteraction andcommunicationamongendusers:cardiologists,patients,andgeneraldoctors.Thekeyfeaturesoftheproposedsolutionare (a)recordingandmanagementofpatients’measurementsofvitalsignsperformedathomeonregularbasis(bloodpressure, bloodglucose,oxygensaturation,weight,andheight),(b)managementofpatien
