Methods:Prospective,comparativecrossoverstudyin14conscious,nonsedated,mechanicallyventilatedadults, duringweaninginauniversityhospitalmedicalintensivecareunit.Patientsweresuccessivelyventilatedina randomorderedcross-oversequencewithneurallyadjustedventilatoryassist(NAVA)andpressuresupport ventilation(PSV).Sleeppolysomnographywasperformedduringfour4-hourperiods,twowitheachmodein randomorder. Results:Thetracingsoftheflow,airwaypressure,andelectricalactivityofthediaphragmwereusedtodiagnose centralapneasandineffectiveefforts.Themainabnormalitieswerealowpercentageofrapideyemovement (REM)sleep,foramedian(25th-75thpercentiles)of11.5%(range,8-20%)oftotalsleep,andahighlyfragmented sleepwith25arousalsandawakeningsperhourofsleep.ProportionsofREMsleepdurationweredifferentinthe twoventilatorymodes(4.5%(range,3-11%)inPSVand16.5%(range,13-29%)duringNAVA(p=0.001)),aswellas thefragmentationindex,with40±20arousalsandawakeningsperhourinPSVand16±9duringNAVA(p= 0.001).Therewerelargedifferencesinineffectiveefforts(24±23perhourofsleepinPSV,and0duringNAVA) andepisodesofcentralapnea(10.5±11inPSVvs.0duringNAVA).Minuteventilationwassimilarinbothmodes.
