胸腰椎爆裂骨折的治疗综述 李定文 郑均华 湖北中医药大学 湖北省第三人民医院 X 关注成功！ 加关注后您将方便地在 我的关注中得到本文献的被引频次变化的通知！ 新浪微博 腾讯微博 人人网 开心网 豆瓣网 网易微博 摘????要： 胸腰椎爆裂骨折是由垂直压缩暴力所致的损伤, 占胸腰椎骨折的50%以上, 伤后椎体粉碎、爆裂, 如椎体后壁骨碎片移位突入椎管易致脊髓或马尾神经损伤。随着现代化工业、交通运输业、建筑业及体育事业的快速发展, 意外事故的发生率也随之上升, 因此胸腰椎骨折目前呈现高发生率、高致残率、高治疗费等特点, 但对其治疗方法的选择目前尚有争议。因此, 选取一个有效而适合的治疗方法对患者的治疗包括减少并发症的发生, 有着重要而深刻的意义。对于不同类型的骨折其治疗方法各不相同, 目前应用于临床的方法有保守疗法、前路手术、后路手术、前后路联合手术及微创手术, 然而最佳治疗方法仍然是不明确的, 手术入路选择一般取决于骨折的类型、骨折部位、骨折时间以及术者对入路熟悉程度。本文对胸腰椎爆裂骨折的治疗进展进行综述。 关键词： 胸腰椎; 爆裂骨折; 治疗; 作者简介：李定文, 硕士研究生, 湖北中医药大学2015级研究生班。 作者简介：郑均华, 主任医师, 湖北中医药大学硕士生导师, 湖北省第三人民医院骨外科。 The Treatment Summarizes of Thoracolumbar Burst Fractures LI Ding-wen ZHANG Jun-hua Hubei University of traditional Chinese Medicine; The third peoples Hospital of Hubei Province; Abstract： Thoracolumbar burst fractures (TLBF) which caused by the vertical compression violence injury, account for more than 50% of thoracolumbar fractures and result in spinal cord or cauda equina nerve damage easily if the fragments of posterior wall of vertebral body displace into vertebral canal. Along with the rapid development of modern industry, transportation industry, construction industry and physical culture, the incidences of accidents also rise. The thoracolumbar fractures present high incidence and high morbidity, high treatment etc. The methods of its treatment are still controversial. Therefore, it has profound significance to choose an effective and appropriate treatment of patients. The management of thoracolumbar burst fractures differs with the types of fractures. Nowadays, the accepted methods include conservative therapy, posterior reduction and instrumentation, anterior decompression and instrumentation, combined anterior-posterior approach and minimally invasive surgery. However, the optimal technique is still not clear. Surgical approach generally depends on the type of fracture, site of fracture, time after injury, and operator acquaintance with approach.
