腹腔镜卵巢囊肿手术中两种止血方法对卵巢储备功能影响 　　【摘要】 目的：探讨腹腔镜卵巢囊肿剥除术中卵巢创面采用双极电凝和缝合两种止血方法对卵巢储备功能的影响。方法：选取双侧卵巢良性囊肿患者160例，根据术中处理卵巢创面的方法不同分为电凝组80例和缝合组80例。比较两组患者术前、术后6个月血清抗苗勒激素（AMH）、卵泡刺激素（FSH）、黄体生成素（LH）、雌二醇（E2）和基础窦卵泡数（AFC），卵巢动脉收缩期峰值血流速度（PSV）的变化，以判断卵巢储备功能。结果：与手术前比较，两组术后FSH、E2均明显升高（P0.05）。术后组间比较，电凝组术后FSH、E2均高于缝合组（P0.05），AMH、AFC、PSV较缝合组均有明显下降（P0.05）。结论：腹腔镜卵巢囊肿剥除术中卵巢创面采用双极电凝和缝合两种止血方法对卵巢储备功能均有影响，尤其电凝止血对卵巢功能的影响更为显著。 　　【关键词】 电凝止血??? 缝合止血； 卵巢储备功能； 腹腔镜手术； 卵巢囊肿 　　【Abstract】 Objective：To explore the influence of laparoscopic ovarian cyst resection of ovary treated with bipolar electrocoagulation and suture in two methods of hemostasis of the ovarian reserve function.Method：160 patients with bilateral ovarian cysts were selected， and they were divided into 80 cases of the electric coagulation group and 80 cases of the suture group according to the different processing method of ovarian wound. The serum anti mullerian hormone （AMH）， follicle stimulating hormone （FSH）， luteinizing hormone （LH）， estradiol （E2） and the number of antral follicle （AFC）， ovarian artery peak systolic velocity （PSV） changed 6 months before operation， postoperative of the two groups were compared， and the ovarian reserve function order were detected.Result：The FSH， E2 of the two groups after the comparison were all increased compare with ovarian function before operation （P0.05）. After the comparison ，the FSH， E2 of the electric coagulation group were higher than those of the suture group （P0.05）， the AMH， AFC， PSV of the electric coagulation group were decreased significantly compared with the suture group （P0.05）.Conclusion：There is influence of laparoscopic ovarian cyst resection of ovary treat with bipolar electrocoagulation and suture of two hemostatic methods on ovarian reserve function ， the effect of electrocoagulation on ovarian function is especially significant. 　　【Key words】 Electric coagulation hemostasis； Suture hemostasis； Ovarian reserve； Laparoscopic operation； Ovarian cyst 　　First-author’s address：Heyuan
