腹腔镜胃癌D2根治术和开腹手术临床疗效对比分析 　　[摘要] 目的 比较腹腔镜胃癌D2根治术与开腹手术治疗进展期胃癌的安全性及近期疗效。方法 对2013年1月―2014年6月在该院普通外科行腹腔镜胃癌根治术（腹腔镜组，56例）和同期行开腹手术（开腹组，60例）患者的临床资料进行回顾性分析，比较两组的手术相关指标、术后恢复情况和肿瘤根治程度。结果 腹腔镜组术中所需时间显著高于开腹组，但其术中出血量、切口长度显著少于开腹组（P0.05）；腹腔镜组术后情况均优于开腹组（P0.05）；开腹组术后并发症发生率（13.33%）显著高于腹腔镜组（3.57%）（P0.05）。结论 进展期胃癌患者经腹腔镜D2根治术治疗后可达到与开腹手术同样的临床近期疗效，且微创优势明显，术后恢复快。 　　[关键词] 腹腔镜手术；进展期胃癌；根治性手术；对比分析 　　[中图分类号] R [文献标识码] A [文章编号] 1674-0742（2015）01（a）-0001-03 　　Comparison of the Clinical Curative Effect Between Laparoscope-assisted D2 Radical Gastrectomy and Open Gastrectomy 　　YUAN Chenghao LI Zeping 　　Department of General Surgery， Sichuan Wusheng Peoples Hospital， Guangan， Sichuan Province， 638400， China 　　[Abstract] Objective To compare the safety and short-term curative effect between laparoscope-assisted D2 radical gastrectomy and open gastrectomy for advanced gastric cancer. Methods A retrospective analysis was conducted on the data of 56 patients（laparoscopic group） underwent laparoscopic radical gastrectomy for gastric cancer and 60 cases（open group） underwent open gastrectomy in the Department of General Surgery of our hospital from January 2013 to June 2014. The indexes related to the operation， postoperative recovery and tumor radical resection extent were compared between the two groups. Results Compared with the open group， the operative time of the laparoscopic group was longer， but the intraoperative blood loss and the length of incision were less（P0.05）； the postoperative conditions of the laparoscopic group were better than those of the open group（P0.05）； the incidence of postoperative complications of the open group（13.33%） was significantly higher than that of the laparoscopic group（3.57%） （P0.05）. Conclusion For advanced gastric cancer， the clinical short-term curative effect of laparoscope-assisted D2 radical gastrectomy is similar to that of open gastrectomy with smaller wound and quicker postoperative recovery. 　　[Key words] Laparoscopic operation； Advanced gastric cance
