PAGE PAGE 10 The effect on corneal endothelium between modern phacomulsification and extracapsular cataract surgery Abstract Objective:To investigate the effect on corneal endothelium in treating hard nuclear cataract by modern phacoemulsification surgery and extracapsular cataract extraction (ECCE). Methods: Totally79 cases (92eyes) of senile cataracr patients were divided into two groups,group 1 and group 2 by treatment modality.Group 1 included 43cases(54eyes),23men,average aged 71years, was treated with phacoemulsification. Group 2 included 36cases(38eyes),19men,average aged 73years was treated with extracapsular cataract extraction (ECCE).Nucleus hardness:IV graph hard nucleus were 49 eyes,Vgraph hard nucleus were5eyes in group 1. IV graph hard nucleus were 30 eyes, Vgraph hard nucleus were 8 eyes in group 2. Preoperative visual acuity was from light sensation to 0.1.Patients were excluded if they had other eye disorders capable of compromising vision,such as high myopia, glaucoma, previous intraocular inflammation or trauma,corneal disease. Patients were also excluded if they were Diabetes mellitus patients.All operation were accomplished by a single surgeon. Central corneal endothelial cell counts were assessed before surgery and 3 months after surgery by a noncontact specular microscope （Topcon SP-3000 noncontact specular microscope and ImageNet analysis system [Topcon, Tokyo, Japan]）. All pictures were captured by the same examiner who was blinded to the surgical technique. The data was analysised by SPSS13.0. Results: 3 months after surgery ,the proportion of visual（≥0.5） was100%、89% in group 1 and 2.There was no significant difference in central corneal endothelial cell density and average size and percentage of hexagonal cells in any preoperative item measured between the two groups.3 month after cataract surgery ,the reduction in cell count in group 1 is 12.9%±4.1% ,in group 2 is 7.3%±3.7% .
