— — PAGE 1— 摘 要 超声冲洗用于慢性牙髓炎一次法 根管治疗术的临床评价 研究生：张蓉 导师：木合塔尔·霍加 摘 要 目的：评价根管超声冲洗技术对成人后牙慢性牙髓炎一次法根管治疗术短期疗 效的影响。方法：选取符合纳入标准的的 62 例患者 62 颗患牙按就诊次序随机分为 2 组：常规注射器冲洗组 30 颗患牙，局麻下开髓、探查根管、确定工作长度后使用 X-SMART 镍钛机用扩大器械冠向下法预备根管。预备过程中用普通注射器吸取 1.25%次氯酸钠溶液和 17%EDTA 溶液各 2ml 反复冲洗根管，最后用注射器向根管内 注射 10mL1.25%次氯酸钠溶液冲洗，吸干并冷牙胶侧方加压法严密充填根管；超声 冲洗组 32 颗患牙，治疗程序同常规注射器冲洗组，常规根管冲洗后根充前再进行根 管内 1.25%次氯酸钠溶液超声冲洗，最后吸干并冷牙胶侧方加压法严密充填根管，观 察随访术后根管充填质量及侧枝根管充填情况、3 天内患牙的疼痛发生情况，3 个月 后治疗疗效。结果：术后即时 X 线拍片证实各组根管充填良好；3 天后复诊了解比 较常规注射器冲洗组和超声冲洗组根管充填后 3 天内疼痛发生情况，差异有统计学 意义(P0.05)，可以认为超声冲洗组的疼痛发生率低于普通注射器冲洗组；根管超声 冲洗后侧支根管充填率无明显提高，差异无统计学意义(P0.05)，尚不能认为两组侧 支根管充填率不同；两组 3 个月后治疗疗效差异无统计学意义(P0.05)，尚不能认为 两组 3 个月后治疗疗效不同。结论：本研究证实超声冲洗应用于成人后牙慢性牙髓 炎一次法根管治疗术可降低术后疼痛发生率；可能提高侧支根管充填率；具有一定 的临床可行性，值得在临床工作中推广应用。 关键词：超声冲洗；慢性牙髓炎；一次法根管治疗术；术后疼痛 新疆医科大学医学硕士学位论文 The clinical evaluation of ultrasonic irrigation applied in one-visit root canal therapy of Chronic pulpitis Master student:Zhang Rong Supervisor: Muhetaer Huojia Abstract Objective: To assess the efficacy of ultrasonic irrigation of the root canal which was applied in one-visit root canal therapy of Chronic pulpitis. Methods: A total of 62 patients with 62 teeth were selected. The teeth were divided into two group ( syringe irrigation group, and ultrasonic irrigation group) randomly according to the doctors office visiting order. The syringe irrigation group 30 teeth proceed access preparation, explored root canal, determine the working length, and prepared the root canal with the crown-down technique in local anesthesia. During the process 1.25% NaOCL (sodium hypochloride) and 17% EDTA were used to irrigate root canal. The ultrasonic irrigation group 32 teeth were used the ultrasonic irrigation after syringe irrigation. In the end, the two groups were obturated the root canal by lateral compaction technique.
