评价三种的方法联合检测对静止型α―地中海贫血诊断价值 　　[摘要] 目的 探讨MCV、亨氏小体和血红蛋白电泳在静止型α-地中海贫血的筛查中的价值。方法 对90例茂名市人民医院2013～2015年确诊的静止型α-地中海贫血患者的结果进行回顾性分析，并对MCV、亨氏小体和血红蛋白电泳单项检测和平行检测及系列检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值进行评价。 结果 MCV、亨氏小体试验、血红蛋白电泳单项检测地中海贫血的灵敏度分别为 35.6%、50%、35.6%，特异度分别为 97.8%、92.2%、96.7%。MCV、亨氏小体试验、血红蛋白电泳平行?合检测的灵敏度和特异度分别为 57.8%、84.4%，系列联合检测的灵敏度和特异度分别为4.4%、100.0%。与各单项试验比较 ，平行联合检测的灵敏度高（P0.05）、系列联合检测的特异度高（P0.05）。 结论 MCV、亨氏小体试验和血红蛋白电泳三者进行平行联合检测可提高灵敏度，三者进行系列联合检测可提高特异度。 　　[关键词] 静止型α-地中海贫血；MCV；亨氏小体；血红蛋白电泳 　　[中图分类号] R556 [文献标识码] A [文章编号] 2095-0616（2017）13-97-04 　　[Abstract] Objective To study the value of the combined tests of MCV， Heinz Body Test and Hemoglobin （Hb） electrophoresis for the diagnosis of static type alpha Mediterranean anemia. Methods Results of 90 cases of stationary alpha thalassemia patients diagnosed in Maoming peoples Hospital from 2013 to 2015 was retrospectively analyzed. The MCV， Heinz Body Test and Hb electrophoresis were individually detected and parallelly detected. The sensitivity， specificity， accuracy， positive predictive value， and negative predictive value of the series of tests were evaluated. Results The sensitivity of MCV， Heinz body test and hemoglobin electrophoresis for individual detection of thalassemia was 35.6%， 50%， 35.6%， respectively. The specificity was 97.8%， 92.2% and 96.7%， respectively. The sensitivity and specificity of parallel combination detection of MCV， Heinz body test and hemoglobin electrophoresis were 57.8% and 84.4% respectively. The sensitivity and specificity of serial combined detection were 4.4% and 100% respectively. Compared with the individual tests， the sensitivity of parallel joint detection was higher （P0.05）， and the specificity of serial joint detection was high （P0.05）. Conclusion The parallel combination detection of MCV， Heinz body test and hemoglobin electrophoresis can increase sensitivity， and three series combined detection can improve the specificity of the three. 　　[Key words] Static type α-Mediterranean anemia； MC
