汕头大 汕头大学医学院硕士学位论文 汕头大 汕头大学医学院硕士学位论文 III III IV IV Abstract Objective To assess the characteristics of left ventricular myocardial motion biomechanics parameters-left ventricular rotation (LVrot) and left ventricular twist (LVtw) by two-dimensional speckle-tracking imaging(STI) and to analyze the change rule of the left ventricular twist(LVtw), geometric morphology parameters-sphericity index (SI) and left ventricular ejection fraction(LVEF) in dilated cardiomyopathy (DCM) and normal person.To study the relationship between left ventricular twist(LVtw), sphericity index (SI) and left ventricular ejection fraction(LVEF),to further understand left ventricular motion biomechanics and geometric morphology changes effect on the systolic function in DCM. Methods 50 patients were including in this study, who were admited to the Affiliated Shantou Hospital of Sun Yat-sen University between the September 2011 and the March 2013. 25 patients were diagnosed as DCM conforming to the WHO 1995 and the Chinese Society of Cardiovascular Disease 2007. In normal control group, 25 pateints who were diagnosed without heart disease by physical and laboratory examination were selected. High frame rate two-dimensional dynamic images of apical four-chamber view, short axis views of the basal and apical level for 3 consecutive cardiac cycles by Philips Sonos IE33 were stored in a hard disk. Displayed the apical four-chamber view，acquired the sphericity index SIs and SId ,and used Simpson`s method to acquire left ventricular end diastolic volume(LVEDV),systolic volume(LVESV) and left ventricular ejection fraction (LVEF). Using the Q-lab 9.0 software to acquire peak apical rotation(PAR) and peak basal rotation(PBR).Peak twist (Ptw) was defined as the maximum difference in rotation angle between the base and apex.
