78O JModUrol，Vo1．19No．12Dec．2014 · 临床研究 · 吉西他滨联合顺铂治疗肾移植术后并发尿路上皮癌的初步探讨田野，肖荆，朱一辰，张建，侯海军，王志鹏，郭宇文，杨培谦 (首都医科大学附属北京友谊医院泌尿外科，北京 100050) Gemicitabinecombinedwith cisplatin forurothelialcarcinoma in renaltransplant recipients TIAN Ye，XIAO Jing，ZHU Yi—chen，ZHANG Jian，HOU Hai—jun，WANG Zhipeng，GUO Yu—wen， YANG Pei—qian (DepartmentofUrology，BeijingFriendshipHospital，CapitalMedicalUniversity，Beijing100050，China) ABSTRACT：Objective ToevaluatetheclinicalefficacyandsideeffectsofintravenouschemotheraDvinrenaltransplant recipientswithurothelialcarcinoma．Methods A totalof7femalerenaltransplantrecipientsaged32—67yearswithconcur- renturothelialcarcinomareceivinggemcitabinecombinedcisplatin (GC)scheme ． 0fallpatients，oflereceivedneoadjuvant chemotherapyandotherschemotherapy afterresection of tumors ． Thechemotherapywas：gemcitabine700— 800mg／m2， ivgtt，dl，d8，dl5，cisplatine50-60mg／m ivgtt，d2．Patientsreceivedthenextchemotherapycycleafter2weeksofinterval andatotalof2to4cycles，andaltogether14cyclesofchemotherapywerefinished． Results Theshortterm toxicityinclud． edhematologytoxicity100 (7／7)，digestivediscomfort71 (5／7)，hair1oss28 (2／7)，andproteinurial4 (1／7)．The functionoftransplantkidneywasimpaired in onepatientafteronecycleofchemotherapy ． During thefollow．up of2 to 11 months，new urothelialcarcinomaappeared in onecasein theoffsideoftherenalpelvisandonepatientdiedofmetastasisin thepsoasmajor．Noobviousabnormalitieswerefoundintheother5patients． Conclusions Renaltransplantrecipients withmuscleinvasiveurothelialcarcinomacanchooseGCschemeofintravenouschemotherapy，buttheeffectofchemothera． peuticdrugsandtheimmune inhibitorsoftenleadsto severemyelosuppression
