精品论文 参考文献 早产儿1岁以内体格发育追赶生长纵向研究 匡晓妮1 张梅2 　　长沙市妇幼保健院 湖南长沙 410007 　　摘要：目的 分析早产儿1岁以内体重、身长、头围追赶生长特征，为早产儿体格生长合理评价提供参考。方法 以本院管理的早产儿为研究对象，生后12个月内定期随访。采用Z评分法对体重、身长和头围进行评估与分析，统计各月龄组三项指标Z值中位数、不同年龄Z值构成比与追赶生长率、△Z值中位数。结果 早产儿体重、身长、头围z值lt;-2构成比均随月龄递增而下降；生后12月追赶率分别为97.6％、87.5％、96.4％；早期早产组体重、身长Z值中位数生后6月大于晚期组，头围Z值中位数生后12月小于早期组；△Z值随月龄递增呈下降趋势。结论 早产儿1岁以内为追赶生长最佳时期，体重、身长和头围的追赶生长呈先快后慢的特征，体重追赶生长优于身长、头围，小胎龄早产儿具有更大的生长潜力。Z评分法是评价早产儿体格生长的适宜方法之一。 　　关键词：追赶生长；早产儿；Z评分 　　Abstract：Objective：To analyze the physical catch-up growth features of preterm infants during infancy in terms of weight，length and head circumference in order to provide reference for the catch-up growth evaluation and clinical intervention.Method：With preterm infants under the management of our hospital as study subjects，a regular follow up is made in the first 12 months after their birth.Z-scoring method is adopted to evaluate and analyze weights，lengths and head circumferences.Statistics is made by Z score medians of three indicators for each age group，Z score composition ratios and catch-up growth rates of different age group，and △Z mean.Results：The composition ratio of the Z score lt;-2（weight，length and head circumference）decreases as the increase of age.The catch-up growth rates of weight，length and head circumferences reach 97.6％，87.5％ and 96.4％ respectively in the first 12 months after birth.Z score medians of weight and length of the early preterm group is greater than late preterm group in the first 6 months after birth，while Z score medians of head circumference is smaller than late preterm group in the first 12 months after birth.△Z decreases as the age increases in preterm infants.Conclusion：The best time of catch-up growth for preterm infants is the first year after birth.The catch-up growth in weight，length and head circumference is featured by growing fast at first and slowing down later.The rate of the catch-up growth in weight is higher than in length and head circumference.Small-for-gestational-age preterm
