Keyword： Home-based elderly care; Combination of medical and elderly care; Community health service; Qufu City; Received： 2017-08-02 人口老龄化问题一直受到社会广泛关注，是学术界关注的重点。由于老年人特殊 的心理、生理特征，以及身体各项机能的退化，加之慢性病成为威胁老年人健康 的最主要疾病，必然会给医疗卫生工作带来-定影响m。我国老年人养老与医 疗服务的传统服务是医、养分离的服务模式。随着经济社会的发展和老龄化进程 的加快，已越来越不适应老年人的卫生与健康需求。为解决养老机构“养老不医 护”、医疗机构“医护不养老”的现实问题，山东省曲阜市政府积极探索一种集 医护、康复、生活照护服务于一体的“居家医康养”服务模式，并将曲阜市鼓楼 社区卫生服务中心作为试点单位，成立了专门的“居家医康养”科，签订了医 养结合服务协议，实现/全科医生、护士、康复师、生活护理贝等专业团队到家 服务。曲阜的做法有效解决了老年人看病难和无人照料的问题，也减轻了社会和 家庭子女的负担，具有良好的社会意义和经济效益。 1背景 2013年9月，国务院印发的《关于加快发展养老服务业的若干意见》M指出，到 2020年全面建成以居家为基础、社区为依托、机构为支撑，功能完善、规模适 度、覆盖城乡的养老服务体系，使生活照料、医疗护理、精神慰藉、紧急救援等 养老服务覆盖所有居家老年人。人UI调查数据显示，2013年我国老年人U总量 已经突破2亿。预计到205
