中青年腰椎间盘突出症行髓核摘除术的远期疗效分析 　　【摘要】 目的：探讨中青年腰椎间盘突出症患者行髓核摘除术后1～5年的远期疗效结果。方法：选取2005年1月-2008年12月在本院就诊的腰椎间盘突出症患者212例，由同一医师按同种方法行髓核摘除术治疗，筛查患者年龄为20～45岁的患者98例为研究对象，对患者术后1、5年进行随访，记录患者对手术疗效的满意度，疗效评价标准为JOA评分、VAS视觉评分，并对比患者手术前后椎间隙高度及脊柱稳定性的影像学资料。结果：手术前患者JOA评分、VAS评分分别与术后5年、术后1年、术后立即检测相比，或术后两两相比差异均有统计学意义（P0.05）。实施髓核摘除术的患者手术前满意度与术后、术后1年、术后5年相比，差异均有统计学意义（P0.05），三者两两满意度比较差异均有统计学意义（P0.05）。结论：对中青年腰椎间盘突出症患者行髓核摘除术，创口小，手术安全，后遗症少，患者恢复能力强，成本低，经1～5年随访，具有较好的远期疗效，值得推广和应用。 　　【关键词】 腰椎间盘突出症； 髓核摘除术； 远期疗效 　　Long Term Efficacy of Youth Lumbar Disc Herniation Underwent Discectomy/CHEN Mao-zhen，LI Zhi.//Medical Innovation of China，2016，13（18）：016-018 　　【Abstract】 Objective：To investigate the long term efficacy of youth and middle-aged lumbar disc herniation underwent discectomy after 1 year to 5 years.Method：A total of 212 cases with lumbar disc herniation from Jan 2005 to Dec 2008 were picked，by the same physician using the same method of nucleus pulposus excision.Screening 98 cases patients age 20 to 45 years old as the study objects，and following-up those patients after 1 year and 5 years and recording surgical curative effect satisfaction，efficacy evaluation criteria via the JOA score，vas visual analogue scale and comparing patients’s operation before and after with intervertebral height and the spinal stability of image data.Result：Preoperative JOA score，VAS score and the operation satisfaction were compared with 5 years，1 year，immediately after the operation，or two phase comparison showed statistical difference（P0.05）.Conclusion：Young and middle-aged lumbar disc herniation patients with nucleus pulposus extirpation，have small wound，safe operation，less complications，patients recover ability，low cost，after 1 year to 5 years follow-up，with better long term efficacy，it is worthy of promotion and application. 　　【Key words】 Lumbar disc herniation； Discectomy； Long term efficacy 　　First-author’s address：The People’s Hospital of Wuchuan City，Wuchuan 524500，China 　　doi：10.3969/j.issn.1674-4985.2016.18
