糖尿病下肢血管急性血栓的超声诊断价值 　　摘要：目的 探讨糖尿病下肢血管动、静脉急性血栓的超声诊断价值。方法 以飞利浦HDII彩色多普勒超声诊断仪对已确诊的糖尿病住院患者28例进行双下肢股，?N，胫前，胫后，足背动脉及同行的深静脉超声检查，观察血管中血栓的部位，大小，形态，性质以及有无侧肢循环，闭塞等。结果 28例糖尿病下肢血管急性血栓住院患者中：6例单侧股动脉急性血栓（其中1例左侧急诊取血栓手术，3例急诊置入支架治疗，1例右侧股深静脉及?N静脉血栓后坏疽--截肢，1例溶栓加生物治疗），4例单侧股深静脉并?N静脉血栓，18例单侧?N静脉血栓，（其中1例右?N静脉血栓并右足坏疸--截肢，1例合并肺栓塞）。结论 超声诊断技术，已成为现代临床医学疾病中常规的诊断方法，所获得的信息在各种医学疾病中占重要地位，尤其是对糖尿病下肢血管急性血栓的定位有独到的临床价值，对早期诊断及选择何种治疗方案（如溶栓，支架，手术及生物治疗）提供主要诊断依据。为今后的随访，康复，提高患者生活质量有重要意义。 　　关键词：糖尿病；下肢血管；急性血栓；超声诊断 　　Abstract：Objective To study the diagnostic value of ultrasound in the diagnosis of acute thrombosis of the lower limbs of diabetic patients. Methods By using Philips HDII color Doppler ultrasonic diagnostic apparatus of undiagnosed diabetes inpatients with 28 cases of double femoral， hamstrings， tibialis anterior， posterior tibial， dorsal artery and peer deep venous ultrasonography， observe the site of plaque in the arteries， size， morphology， properties and limb circulation， occlusion and so on. Results 28 cases of hospitalized patients with acute thrombosis of diabetic lower extremity vascular： 6 cases of unilateral femoral acute arterial thrombosis （which 1 case left emergency removal of thrombus operation， 3 cases of emergency stenting treatment， 1 case right femoral deep vein and popliteal vein thrombosis after gangrene， amputation， 1 cases of thrombolysis and biological treatment）， 4 cases of unilateral femoral deep vein and popliteal vein thrombosis， 18 cases of unilateral popliteal vein thrombosis （which 1 case right popliteal vein thrombosis and right foot gangrene -- amputation， 1 case complicated with pulmonary embolism）. Conclusion Ultrasound diagnosis technology has become a routine diagnostic method in modern clinical medicine. The information obtained is important in all kinds of medical diseases， especially for the treatment of acute thrombosis of lower extremities.
