滋阴补肾化瘀法联合中药熏蒸治疗高血压肾损害临床研究 　　[摘要]目的 观察中药内服联合外治中药熏蒸治疗高血压肾病的效果。方法 将临床确诊为高血压肾损害的78例患者按照治疗方法分为治疗组（42例）与对照组（36例）。对照组除降压外不予其他处理；治疗组采用降压+滋阴补肾化瘀药内服+中药熏蒸治疗。比较两组的治疗效果。结果 两组治疗后的中医症候积分改善，治疗组优于对照组（P0.05），尿微量蛋白、NAG酶、血清胱抑素C（Cys-C）均下降，治疗组优于对照组（P0.05）。与治疗前比较，两组治疗后的血清CRP、内皮素（ET）水平下降，两组治疗后的上述指标比较，差异有统计学意义（P0.05）。结论 滋阴补肾化瘀法联合中药熏蒸能改善高血压肾损害患者的中医症候，降低尿蛋白，改善肾损害，其作用机制可能与改善血管ET、减轻炎症反应相关。 　　[关键词]滋阴补肾化瘀；中药熏蒸；高血压；肾损害 　　[中图分类号] R544.1 [文献标识码] A [文章编号] 1674-4721（2017）12（a）-0151-03 　　[Abstract]Objective To observe the effect of oral administration of traditional Chinese medicine combined with external application of traditional Chinese herbal fumigation treating hypertensive nephropathy.Methods 78 patients with definite diagnosis of hypertension-induced renal injury were divided into the treatment group （n=42） and control group （n=36） according to the different therapeutic method.In the control group，no other treatment except antihypertension was used，while the treatment group was given antihypertension，oral administration of nourishing yin，tonifying kidney，and removing blood stasis drugs，and traditional Chinese herbal fumigation.The therapeutic effect was compared between the two groups.Results After treatment，score of symptoms in traditional Chinese medicine were both improved in the two groups，and the treatment group was better than of the control group （P0.05）.Urinary microprotein and NAG enzyme，and serum cystatin C （Cys-C） were all decreased，and the treatment group was better than of the control group （P0.05）.The level of serum C-reactive protein （CRP） and endothelin （ET） were both decreased after therapies compared with those before treatment，and there was a statistical difference of the above-mentioned indexes after treatment between the two groups （P0.05）.Conclusion Nourishing yin，tonifying kidney，and removing blood stasis therapy combined with traditional Chinese herbal fumigation can improve TCM symptoms in patients with hypertension-induced renal injury，decrease urine protein and be
