Background.ItisunclearifCD4cellcountsatHIVdiagnosishaveimprovedovera10-yearperiodofexpandedHIVtestingin theUSA.Methods.WestudiedHOPSparticipantsdiagnosedwithHIVinfection≤6monthspriortoentryintocareduring2000– 2009.WeassessedthecorrelatesofCD4count200cells/mm atHIVdiagnosis(lateHIVdiagnosis)bylogisticregression.Results. 3 Of1,203eligiblepatients,936(78%)hadaCD4countwithin3monthsafterHIVdiagnosis.MedianCD4countatHIVdiagnosis was299cells/mm 3 anddidnotsigni?cantlyimproveovertime(P = 0.13).Comparingperiods2000-2001versus2008-2009, respectively,39%and35%ofpatientshadalateHIVdiagnosis(P = 0.34).IndependentcorrelatesoflateHIVdiagnosiswere havinganHIVriskotherthanbeingMSM,age≥35yearsatdiagnosis,andbeingofnonwhiterace/ethnicity.Conclusions .There isneedforroutineuniversalHIVtestingtoreducethefrequencyoflateHIVdiagnosisandincreaseopportunityforpatient-and potentiallypopulation-levelbene?tsassociatedwithearlyantiretroviraltreatment. 1.Introduction The CDC has been promoting strategies to encourage morewidespreadHIVscreeningtodiagnoseinfectedpersons earlierinthecourseoftheirillness,including
