上海师范大学硕士学位论文 摘要 万方数据 万方数据 摘要 和世界上其他国家一样，中国在人口老龄化加剧、人口与家庭结构的转变、福 利制度滞后于社会发展、老年人疾病慢性化、照护内容复杂化、以及双薪家庭增多 等大背景下，传统的老年人长期照护功能进一步弱化，老年人的长期照护问题备受 关注。为了解决老年人长期照护问题，学术界提出不同的老年人长期照护服务供给 模式，包括以“国家、市场、家庭”为主的福利三角模式、有以“社区+家庭”相 结合的居家养老模式、以及有以“政府、社区、家庭、专业机构”相结合的四位一 体模式等。本本基于福利多元主义、社会支持和交换理论视角，老年人长期照护服 务供给问题方面进行探讨。 本文是基于 S 市中人口老龄化最严重的 J 区为研究案例展开问卷调研，通过 SPSS 统计软件分析：S 市 J 区老年人健康现状和照护服务需求调查，包括老年人医 疗保健的需求、经济支持的需求、情感支持的需求、社区医疗康复的需求、社区生 活照料的需求和老年人在家养老与照顾的需求。同时通过文献分析、问卷调查、统 计分析等方法阐释 S 市 J 区的老年人长期照护服务供给内容，包括；医疗保健、文 化娱乐、生活照料、精神慰藉、安全保障和法律援助等内容；最后分析 S 市 J 区老 年人长期照护服务供给的特点与不足。 本文通过以 S 市老龄化最严重的 J 区作为研究案例，是理论与应用相结合的研 究；分析以“在地老化”理念为主、生活照料与专业护理有效结合、形成了一个较 为完整的服务链的“乐龄生活圈”计划中，政府、社会力量、社区、家庭、养老机 构五者之间是如何联动和衔接给老年人长期照护提供服务。本文结合具体的调研情 况和对已有的文献梳理的基础上提出：“五位一体”老年人长期照护服务供给体 系，强调：除了政府、社区、家庭、养老机构等四位以外，“第五位——社会力 量”对老年人长期照护服务供给的影响。本研究主要创新之处在于：在重新较为系 统梳理了老年人长期照护服务供给体系研究的前提下，站在老年人服务支持体系层 面，重新归类划分老年人服务供给主体的类型，分为了政府、社区、家庭、养老机 构、社会力量等五大主体，提出了由该五大主体组成的“一辕协调、四轮驱动”关 系的老年人长期照护服务供给体系。 关键词： 老年人；长期照护服务；供给；模式；五位一体 I  Abstract Shanghai Normal University Master of Management Abstract Like the rest of the world, due to aging population increasing significantly, changing in population and family structure, social welfare system lagging behind the development of society, sophisticated demands for long-term care of elder of chronic medical conditions, increasing of double-income families, the traditional long-term care of elder capabilities become weaker. Academia struggle with concerns related to the serious long- term care of elder’s service burden. There are three major models of long-term care of elder’s service structure. The first one is welfare triangle model which bases on the relationship between nation, market and family mainly. The second is home base-on long term care where the foundation is the combination of community and family.
