人工股骨头置换、全髋关节置换治疗股骨颈骨折的疗效比较研究 　　[摘要] 目的 比较人工股骨头置换、全髋关节置换治疗股骨颈骨折的临床疗效。方法 选择2010年12月～2013年12月我院行人工股骨头置换与全髋关节置换术治疗的股骨颈骨折患者60例，其中行人工股骨头置换术共32例，设立为A组，行全髋关节置换术28例，设立为B组。 结果 A组患者的手术时间（80.5±12.2 ）min、术中输血量（219.6±53.2） mL，分别短于B组或少于B组，组间比较，差异有统计学意义（P0.05）。B组患者的有效率达85.7%，与A组的有效率比较，差异有统计学意义。其中A组并发症发生率达43.75%（14/32），B组并发症发生率达28.57%（8/28），两组比较，差异有统计学意义（P0.05）。其中A组早期并发症与B组比较，差异无统计学意义，但B组晚期并发症发生率达17.86%（5/28）， 明显少于A组31.25%（10/32），差异有统计学意义（P0.05）。 结论 人工股骨头置换术具有操作相对简单、创伤小、出血少等优点，而全髋关节置换具有下床活动早，疼痛轻、且髋关节功能恢复快、晚期并发症少等优点。 　　[关键词] 股骨颈骨折；人工股骨头置换；全髋关节置换；并发症 　　[中图分类号] R683.42 [文献标识码] B [文章编号] 1673-9701（2014）12-0045-03 　　[Abstract] Objective To compare clinical efficacy of hemiarthroplasty，total hip arthroplasty in treatment of femoral neck fractures. Methods Selected 60 cases of hemiarthroplasty with total hip arthroplasty for femoral neck fracture patients from December 2010 to December 2013 in our hospital，60 cases patients with femoral neck fractures，hemiarthroplasty of 32 cases（group A），total hip arthroplasty 28 cases（group B）. Results A group of patients operative time 80.5±12.2 min， intraoperative blood transfusion 219.6 ± 53.2 ml，respectively，was shorter than group B，the difference was significant between two groups（P0.05）. B group of patients with the effective rate of was 85.7%，was higher than group A，the difference was significant between two groups（P0.05）. The complication rate of Group A was 43.75%（14 /32），the complication rate of group B was 28.57%（8/28），the difference was significant between two groups（P0.05）. The late complication rate of group B was 17.86%（5/28），was significantly less than group A[31.25%（10/32）]，the difference was significant between two groups（P 0.05）. Conclusion Hemiarthroplasty in threatment of with relatively simple，less invasive， less bleeding，etc.，and total hip arthroplasty with early ambulation，pain，and faster recovery of hip function，fewer late complications. 　　[Key words] Femoral neck fracture；Hemiarthroplasty； Total hip arthroplasty； Complicati
