· 374· 塞旦蕉疸垒：墨 生!旦第23 第4期 The Practical Journal ofCancer ． Ju1v 2008．v0l 23。N0．4 食管癌常规放疗与三维适形放疗剂量学比较研究 马代远 柳 弥 谭榜宪 李贤富 【摘要】 目的 比较食管癌常规二维放疗(2DRT)与全程三维适形放疗(3DCRT)肿瘤靶区和周围敏感器官剂量分 布。方法 l5例中晚期食管癌患者行CT模拟定位，CT扫描图像经网络传输到治疗计划系统(TPS)进行三维重建，勾画 大体肿瘤体积(GTV)、临床靶体积(CTV)和危及器官(OARs)。每例做2DRT和3DCRT 2套计划。计划评估、优化后比 较两者的剂量分布情况。结果 (j)2DRT和3DCRT的GTV—V 0。分别为30．1％、38．4％；GTV．V。 分别为83．2％、94．2％， CTV—V95分别为75．2％、90．2％ ； V—V95分别为65．2％、83．2％ ；②2DRT和3DCRT计划的两肺v∞分别为18．1％、 29．4％，脊髓V4o分别为50．1％、26．4％，心脏V45分别为17、2％、20．1％。结论 3DCRT技术比2DRT技术更能够给予 食管癌临床靶区均匀的剂量分布，并使脊髓得到保护，两肺和心脏的受照在耐受范围内。 【关键词】 食管肿瘤；放射治疗；常规放疗；三维适形放疗 中图分类号：R730．55；R735．1 文献标识码：A 文章编号：1001—5930(2008)04-0374-04 Dosimetrical Comparison of Conventional and Three Dimensional Conformal Radiotherapy for Esophageal Cancer MA Dai—yll,an，LIU胁，TAN Bang—xian，el a1． Department ofRadiotherapy，The First Affiliated Hospital。North Shuan Medal College，Nanchong，637000 【Abstract】 Objective To compare the dose distribution in the prima~lesion and surrounding normal tissues between the conventional 2-dimensional radiotherapy(2DRT)and 3-dimentional conformal radiotherapy(3DCRT)for esophageal cancer． Methods 15 patients with locally advanced esophageal carcinoma were included in the study．After CT scanning，the images were transferred to the treatment planning system(ADAC Pinnacle 7．6c)，and gross tumor volume(GTV)，clinical target volume (CTV)and organs at risk(OAR)were 2DRT and 3DCRT plans were designed for each patients to compare the dose distribu— tion．Results 0)The volumes of GTV covered by 100％prescription dose were 30．1％and 38．4％for 2DRT and 3DCRT．re— spectively，and the volumes of GTV，CTV and V covered by 95％ prescripfion dose were 83．2％ and 94．2％ ，75．2％ and 9
