实用医学影像杂志 2017,05(18),396-398 DOI:10J6106/j.cnki.cnl4-1281/r.2017.05.010 多层螺旋CT在结肠癌术前诊断及分期 中的应用价值分析 全国彪张康胜温志玲欧春燕陈俏倩 广东省湛江市第二人民医院医学影像科广东省湛江 市第二人民医院药剂科 导出/参考文献关注 分亨 收藏打印 摘要： 目的分析多层螺旋CT (MSCT)在结肠癌术前诊断及分期中的应用价值。方法选 择2013年8刀至2016年8刀经我院手术病理确诊为结肠癌患者78例，所有患 者术前均接受MSCT扫描检查。以手术病理检查结果为金标准，评估MSCT诊断浆 膜外侵犯、淋巴结转移的灵敏度、特异度、准确度，分析结肠癌MSCT特征及分 期的准确性。结果患者肠壁呈不规则增厚，或呈不规则形或分叶状肠腔肿块。 MSCT诊断浆膜外侵犯与淋巴结转移的灵敏度、特异度、准确度分别为97%、85%、 94%;65%、96%、86%o术前MSCT分期结肠癌的总体准确度为95%。结论MSCT在 结肠癌术前诊断与分期中具有显著的应用价值，能够为临床制定合理的手术方 案提供指导。 关键词: 结肠肿瘤;体层摄影术，螺旋计算机;诊断; 收稿日期:2017-06-20 Application value of multi-slice spiral CT in preoperative diagnosis and staging of colon cancer Quan Guobiao Zhang Kangsheng Wen Zhiling Ou Chunyan Chen Qiaoqian Imaging Room, The Second Hospital of Zhanjiang, Guangdong; Abstract： Objectivc To analyze the application value of multi-slice spiral CT (MSCT) in preoperative diagnosis and staging of colon cancer. Methods Seventy-eight patients confirmed with colon cancer by operative pathology in out hospital from August 2013 to August 2016 were selected, and all of them were given MSCT scanning. The op_erative and pathological findings were taken as gold steindard The sensitivity，specificity and accuracy of MSCT in di-agnosis of serosal invasion and lymph node metastasis were assessed. The MSCT characteristics of colon cancer and accuracy of staging were analyzed. Results The intestinal wall of patients showed as irregular thickening, or irregular shape and lobulated intraluminal mass. The sensitivity， specificity emd accuracy of MSCT in diagnosis of serosal inva-sion and lymph node metastasis were 97%, 85%, 94%; 65%, 96%, 86%. The overall accuracy of preoperative MSCT staging of colon cancer was 95%. Con elusion MSCT in preoperative diag nosis and staging of colon cancer has significant application value, which can provide guidanee for making reasonable operation plan.
